Peri-menopause and Menopause encompasses the time wherein ovarian activity diminishes, instigating symptoms of hormone fluctuations until permanent cessation of menses.  Menopause, whether artificially induced through surgery or other conditions, indicates a loss of ovarian function.  Declining ovarian eggs and follicles, reduced ovarian hormone production including estrogen, progesterone, and testosterone, and increased/fluctuating levels of follicle stimulating hormone (FSH) and luteinizing hormone (LH) contribute to the dreaded menopausal symptoms including hot flashes, night sweats, mood alterations, menstrual irregularities, fatigue, atrophy of vaginal tissues, reduced libido, sleep disturbances, and changes in memory and cognitive function. Osteoporosis and cardiovascular disease risk increases with declining ovarian hormone levels.

Symptoms of flushing and sweats affect 70% of menopausal women, while sleep disruptions plague 40-60%.  Conventionally, Hormone Replacement Therapy (HRT) was recommended due its effectiveness for these symptoms and to minimize postmenopausal bone loss.  Women’s Health Initiative multi-center study in 2002 uncovered some disturbing health risks for cancer and cardiovascular disease in patients using synthetic Premarin and/or Progestin.

This HRT cautionary issue prompted interest in safer, non-hormonal treatments.  Significant evidence has arisen since then to support the use of many botanical and nutraceutical approaches which also provide the full-spectrum of treatment minimizing climacteric complaints, and reducing risk for heart disease and osteoporosis.

DONG QUAI  Angelica sinensis acts to enhance estrogen regulation, rather than being directly estrogenic. Traditionally, it was used as a blood tonic, to enhancing the circulation and provide energy, vitality, and improved resistance to disease. Dong quai Angelica sinensis has been used in traditional Chinese medicine for more than 2000 years.  In the Chinese Materia Medica, Dong quai is indicated for disorders of menstruation including menopausal symptoms such as menstrual flow abnormalities, dizziness, and insomnia.

CHASTE TREE  Vitex agnus-castus, indigenous to Mediterranean countries and Asia, has been renown as both aphrodisiac and anaphrodisiac.  Suppressing sexual desire in celibate monks in the Middle Ages, it was know as Monk’s pepper.  In the 20th century, Chaste Tree is known as Female “Viagra”.  Chaste Tree has the ability to regulate and stimulate the anterior pituitary gland, normalizing the body’s amount of progesterone.  German researchers discovered its ability to bind dopamine receptors in the anterior pituitary, decreasing prolactin secretion, increasing progesterone in the luteal phase of the cycle.  By increasing LH, Chaste Tree helps to lengthen the luteal phase of the menstrual cycle.  Pituitary secretion of LH signals ovarian egg release, and the empty follicle (corpus luteum) manufactures the monthly progesterone surges.

BLACK COHOSH Cimicifuga racemosa indigenous to Eastern US & Canada, contains many biologically active constituents, primarily triterpene glycosides, flavonoids, phenolic acids, and tannins.  Black Cohosh is approved by the German Commission E for treatment of climacteric (menopausal) complaints and premenstrual and dysmenorrheic disorders. Symptoms including hot flashes, night sweats, vaginal dryness and mucosal thinning and sleep/emotional disturbances have shown dramatic improvement with use of Black Cohosh, reducing only Luteinizing hormone levels (LH), but not Follicle Stimulation Hormone (FSH).  Black Cohosh may assist in post-menopausal osteoporosis, by preventing bone loss, and has shown to stimulate bone formation in ovariectomized rats.

Cimicifuga racemosa may potentiate Tamoxifen’s effects and show no stimulatory effects on estrogen receptor positive cells.

RED CLOVER  Trifolium pratense provides phytoestrogen isoflavones (genistein, daidzein, formononetin, and biochanin). Asian, Mediterranean, and Central American peoples show typical dietary intake of these isoflavones, which show antioxidant actions, enzyme inhibition, and regulation of various steroidal reproductive hormones. Trifolium isoflavones have been shown to reduce the frequency and severity of hot flashes, promote emotional well-being, and assist in bone health maintenance.

Traditionally, Red Clover Trifolium pratense has been used for inducing menstruation, aiding change of life in women, or used as a fertility tonic. Isoflavones in Red Clover have been found to be the main therapeutic compound in the plant for the treatment of menopausal symptoms.

GAMMA-ORYZANOL is found in grains and isolated from rice bran oil.  Gamma-oryzanol, consists of triterpene alcohol and phytosterol esters of ferulic acid, and reduces elevated levels of FSH, LH and prolactin (PRL) following ovariectomy in rats. According to Kupperman’s index of menopausal symptoms, the symptoms of climacteric disturbance were successfully treated in 85% of the cases.  Gamma-oryzanol has also shown antioxidant, neuroendocrine, anabolic, anti-ulcerogenic, anti-thrombotic, and anti-hyperlipidemic properties.

NORWAY SPRUCE Picea abies contains HMRlignan™, a class of phytoestrogen from plants which share structural similarities with mammalian estrogens.  Flaxseed is a well-known source of mammalian lignan precursors.  HMRlignan™ is a naturally occurring plant lignan, 7-hydroxymatairesinol (7-HMR), from the Norway spruce Picea abies.

In humans, 7-HMR is a direct metabolic precursor of the mammalian lignan, enterolactone (ENL). Unlike dietary lignans, which exist as glycosides and must be cleaved within the intestinal tract, HMRlignan™ is delivered in its pure aglycone form for more rapid and efficient transformation into bioactive enterolactone. Pharmacokinetic data in both postmenopausal women and young men have clearly shown that HMRlignan™ significantly increases blood levels of ENL.  When taken daily, it has been shown to provide an ongoing increase in blood levels of ENL in 4 to 8 week clinical studies.

Dietary lignans are converted by gut microflora to the mammalian lignans enterolactone (ENL) and enterodiol (END).  HMR is directly converted to ENL while pinoresinol, lariciresinol, and secoisolariciresinal are first converted END, which then partially converts to ENL. ENL is the primary circulating mammalian lignan and is used as the primary biomarker for plant lignan intake.

The enterolignans (ENL) are absorbed through the gut wall, and work to modulate estrogenic activity through estrogen receptor (ER) interaction.  Similar in effect to other phytoestrogens, lignans balance estrogen by antagonizing ERs when estrogen levels are high, and promoting ERs as weak agonists when estrogen is low.  Estrogen balance is also influenced by lignans as they inhibit aromatase activity, and shift estrogen metabolism to minimize harmful 16-alpha-hydroxyestrone metabolites.

Large population studies have found that higher blood levels of ENL promote cardiovascular health, breast health in women, and prostate health in men. Studies also suggest that supplementation with lignans may help reduce hot flashes associated with menopause.

Pharmakinetic studies show HMRlignan™ was rapidly absorbed.  Plasma concentrations increased with increasing dose and the increases in AUC and C(max) values appeared to be linear. The apparent mean elimination half-life ranged from 2.6 to 5.1 hours. ENL was detected as the main metabolite and was dose-dependently elevated – doses as low as 10 mg produced a marked elevation in ENL concentrations.

Significant research has found that increased dietary lignan intake (such as hydroxymatairesinol 7-HMR), as well as increased blood levels of enterolactone (ENL), is associated with improved breast health in women.  The protection appears to extend all the way from young adulthood through the postmenopausal phase of a woman’s life. Large population studies have found that higher intakes of lignans and especially higher blood levels of enterolactone are associated with promoting breast health in both premenopausal and postmenopausal women.

In addition to breast and heart health, studies indicate that lignans may afford other health benefits to women during menopause and beyond.  Research in the Netherlands reported that during the postmenopausal phase of a woman’s life, mental sharpness and focus were found to be improved with increased lignan intake in the diet.
